When Conservative Care (PT) Deserves More Time

Unless you are dealing with a medical emergency or progressive neurological deficits, rushing into invasive procedures skips a crucial diagnostic and therapeutic window. For the vast majority of non-emergency orthopedic conditions, a dedicated 6- to 12-week trial of structured physical therapy isn’t just a hurdle to jump, it is the clinical benchmark to truly understand what is driving your symptoms.

Pain is often surprisingly decoupled from structural findings on an MRI; a mild labral tear or meniscus wear might be present on paper, but the actual source of your pain is frequently the compensation patterns and improper joint loading happening around it. If targeted physical therapy can correct those movement dysfunctions, redistribute the stress, and calm down the surrounding tissue, the pain often vanishes entirely. When you fix the way you move, you frequently discover that an anatomical tear on a scan never required a surgical fix in the first place.

Red Flags: When to Refer Directly to a Surgeon

One of the most important parts of my job is recognizing when regenerative medicine is not the appropriate treatment. Our goal is not to dismiss surgery outright. It’s to help each patient find the treatment that gives them the best long-term outcome. 

Some conditions require evaluation by an orthopedic surgeon rather than regenerative treatment alone. These include: 

  • Progressive neurological deficits, such as new weaknesses, foot drop, or loss of bowel or bladder control.  
  • Complete or high-grade tendon and ligament ruptures, particularly in younger or highly active individuals.  
  • Unstable fractures or traumatic injuries requiring surgical fixation.  
  • Advanced structural damage where restoring normal function is unlikely without surgical repair.  

Every patient undergoes a comprehensive musculoskeletal evaluation, including diagnostic ultrasound, x-ray, and MRI when appropriate. If surgery offers the best opportunity for recovery, we recommend a surgical consultation rather than proceeding with regenerative treatment. 

Regenerative medicine should never replace necessary surgery. It serves as an important option for patients who fall between conservative care and surgery—or as a valuable complement before or after surgery when appropriate.

The Middle Ground: Escalating Care with Maple Medical

Many patients believe their only options are to continue living with pain or undergo surgery. There is often an important middle ground.

I had a stem cell procedure done by Dr. Blatz on one of my knees. Long story short, I had a long history of degenerative issues. I had done lots of PT, am too young for certain other interventions, but it still continued to worsen. After the procedure done by Dr. Blatz, I was able to do simple things like sit and stand and walk on stairs with no pain in my knee (this was after having nearly 10 years of pain).
